Tennessee (C-42)
Employee’s Choice of Physician form. Employees select up to 3 physicians from the employer’s panel, plus an optional telehealth-only provider. The system validates selections and generates the completed C-42 PDF automatically.
Georgia (WC-P1)
Posted Panel of Physicians form. Employers select 6–9 physicians with at least one orthopedic surgeon and no more than two industrial clinics. Physicians are grouped by practice on the form, and the completed WC-P1 PDF is generated instantly.
